COVID Vaccination Clinic at McKinley High School on 5/15/21 - McKinley High School COVID Vaccination Clinic Date May 15, 2021 with follow up clinic for second shot on June 5, 2021... Time 10:00am - 2:00pm Location McKinley High School Vaccine Pfizer Eligibility Anyone 16 years or older. If the FDA approves the vaccine for younger children before May 15, then anyone 12 years and older will be accepted as well. Registration Pre-registration is highly recommended to minimize wait time on the day of the clinic. Registration can also be done on-site. Walk-ins will be accepted. 1. Complete the Queens Registration Form and Consent Form (if you are under the age of 18). 2. Scan or take a picture of the forms and send them to [email protected] by May 12. 3. You will be sent a confirmation email with an appointment time. Insurance is NOT required for vaccination. Open to Public Yes! Please encourage all of your family and friends to get vaccinated at McKinley's clinic. Pre-registration is highly recommended but not required.
